What Is a Plan of Care (POC)?
A Plan of Care (POC) is a structured, written document developed by your healthcare team that outlines the supports, services, and strategies tailored to your health goals. Using a person-centered approach, each POC ensures that your treatment is individualized, measurable, and effective.
What Does a Health Hive POC Include?
Each Health Hive POC includes 8 visits and is carefully designed to provide:
Personalized goals and objectives based on your condition and lifestyle
Detailed intervention strategies created by our experienced health professionals
Defined frequency and timelines to monitor your progress and adjust care as needed
The number of POCs you may need depends on the complexity of your health issue—from low complexity (e.g., mild arthritis) to high complexity (e.g., chronic or multi-system conditions).
Key Details:
Plans of Care require a full investment—giving you full access to your healthcare team, for 8 visits.
Unused visits are honored for tune-ups—for example, if you only need 4 visits to reach your goals, you’ll have 4 visits remaining to use as tune-ups.
Payment plans are available—we believe finances shouldn’t be a barrier to optimal health.
Superbills are provided upon request—after each paid visit, you’ll receive a detailed bill that you can submit to your insurance provider for potential reimbursement.
