Third, Health Hive doctors develop carefully constructed plans of care to best serve our patients.

What is a Plan of Care?

A Plan Of Care, commonly known in medicine as POC, means “a written document developed for each patient by the support team using a person-centered approach that describes the supports, services, and resources provided or accessed to address the needs of the individual.”

Health Hive POC:

Health Hives offer POCs which contain 8 visits per POC. Each POC contains specific tasks and activities to be carried out by our health care providers, including, but not limited to, patient-specific goals and objectives, detailed descriptions of the interventions, frequency and time frames for ongoing services. Our recommendations for the number of plans of care vary based on the complexity of the health problem, ranging from low to high complexity. Plans of care are purchased in full. Visits within a plan of care are not reimbursed and do not expire. Meaning, if you have a low complexity case (ie. knee pain from arthritis) and purchase one plan of care (8 visits), but it only takes you four (4) visits to resolve symptoms and return to prior level of function - you have four (4) remaining visits to use, forever. Payment plans are available to help make our patient’s financial responsibility manageable for any budget. After a paid visit, the clients are able to receive a detailed bill, called a superbill, which can be submitted by clients for reimbursement from their insurance provider, if they so choose.
